I'm guessing it must have been human error cause these choppers are doing pretty good in the army.Any info in regards to cause of crash in equador's presidential chopper?

this ties in exactly what I had heard from a senior IAF pilot operating in that area. ALH is simply not fit for these altitudes. @Kunal Biswas any comments on this.the last nine months have seen at least two "category one" crashes at the glacier that have gone unreported and have raised serious concerns.Both the crashes involved the indigenous Advanced Light Helicopters (ALH) that had been inducted by Army Aviation to work well beyond their design capacity to supply troops at high altitudes. In August, a chopper crashed while landing close to the Amar helipad on the Siachen Glacier and went down a crevasse. Similarly in March this year, another ALH went down on the northern glacier while landing at a narrow helipad.
"The two accidents occurred at a time when the choppers were taking off or landing at extremely narrow airfields where even a freak wind can cause havoc. Thankfully, in both cases, the pilots managed to jump out in time and only got injured. Both aircraft are damaged beyond repair and one cannot even be recovered," said an Army official on condition of anonymity.
Mk-1 and Mk-2 don't have much juice left at those altitudes, it's Mk-3 which can fly safely at those altitude. Most Dhruv in service are M-1 and Mk-2. Last AERO India, the IAF dignitaries were complaining Mk-1 and Mk-2 had a major fault which used to cause category-1 crashes and HAL in all it's lethargy could only fix a handful of helicopters several years after the problem was identified and the turn around time for maintenance of Dhruv was 8-9 months.Bad news for ALH. 2 ALH helicopters have crashed in Siachen ruling it out for this job.
