It's hard to believe this can happen on a train as modern as WAG 12.

It has mechanisms that require pressing a switch every 2 minutes, and if that's not done, the train automatically halts.

Reports would never be made public.
Could be that the mechanics was turned off somehow.
Maybe the mechanism broke down long ago and the locomotives was doing without it.
Maybe the driver dozed off at the perfect moment and the mechanism could not account for it.
