Oxygen saturation



Amid states facing a shortage of medical oxygen, the doctors stressed that it should be used judiciously; every patient may not need it. “We have enough oxygen if we use it judiciously. I want to tell the public that if you don’t need oxygen then don’t use it as a security blanket. Waste of oxygen will only lead to depriving someone who needs it,” Dr Naresh Trehan, chairman, Medanta, remarked.


One need not worry if the oxygen saturation is above 94 per cent, Dr Shetty assured. “But if it is falling after exercise, then you must call a doctor. It’s important you get the right treatment at the right time.”

Hospitalisation


The medical experts emphasised that every COVID-19 patient need not rush to the hospital. “Unnecessary rushing to the hospital will only lead to chaos… Most of the people will recover without hospitalisation. Don’t rush to hospitals because in doing so you are denying someone more serious of his or her right,” Dr Guleria stated.


Dr Trehan also added that there is “no such thing as preventive hospitalisation”. “Less percentage of the population requires hospitalisation. The hospital beds should be utilised judiciously. We must take responsibility for this.”

Remdesivir


More than 85 per cent of patients will recover without steroids or serious medicines like Remdisivir and other drugs, Dr Guleria stressed. He said that only a certain per cent of patients require convalescent plasma.


“We’ve now made a protocol that Remdesivir is to be given not to everyone who tests positive. Only after a doctor has looked at test results, symptoms, comorbidities, it’s to be given. Remdesivir isn’t a ‘Ramban‘,” Dr Trehan mentioned.


COVID vaccine


With reports of some people testing COVID-19 positive even after the second dose of the vaccine, some doubts have been raised on its efficacy. Dr Guleria, however, said, “People say what is the use of getting COVID vaccine because we know someone who had taken the vaccine but still got COVID-19. I want to reiterate that you can still get the virus but it will lessen the severity of the virus.”


Even after being vaccinated one should continue to follow COVID-19 protocol and wear masks. “The vaccine prevents you from getting the disease in the form of severe illness. It may not prevent you from getting the infection. Important to understand that even after the vaccine we may have a positive report, it is why important to wear a mask even after the vaccine.”

Whiff of Hope: Why India Needn't Worry about Medical Oxygen and Ventilators during Covid-19

According to Saket Tiku, India has enough supply of oxygen in stock. The demand has gone down post the advent of Covid-19 by at least 20-25% as most non-essential surgeries have been deferred. The lockdown has also meant that supply to the industry has been reduced which in turn implies that more gas is available to be converted to medical oxygen. He added that India currently has 80,000 tonnes of oxygen in stock while consumption of medical oxygen is approximately 700 tonnes per day.

Oxygen is the largest revenue generating industrial gas in India. Also, only 14% of the reported cases need medical oxygen and given the low number of cases in India (compared to Europe and USA) and with the rate of growth only slowing down post the stringent lockdown, the country need not panic.

Gasworld also states that in most circumstances, ASUs (air separation plants) tend to operate at around 75-85% of their maximum capacity. This means that in the context of any significant outbreak of Covid-19, the output of oxygen can be increased without the need for creating new plants.

Meanwhile, India continues to devise new innovative ways to meet the challenges of any future medical oxygen emergency. The naval dockyard in Visakhapatnam has designed an innovative 'Portable Multi-feed Oxygen Manifold (MOM)' using a six-way radial header fitted to a single cylinder. This innovation would enable one oxygen bottle to supply to six patients simultaneously.

Ventilators are required in acute cases which roughly make up 5% of the total affected cases. India, at the end of March, had around 40,000 ventilators. This would mean that the total number of reported cases would need to touch 8,00,000 for all these 40,000 ventilators to be put for Covid-19 use. India currently stands at approximately 19,000 reported cases.

They are also of the view that the new cases being reported daily should be categorised as mild, moderate or severe to better understand the pandemic.
In a recent study, Mavalankar found that between March and July, the percentage of deaths due to COVID-19 compared to other diseases in India was 1.3, as against 13 per cent in the US and 17.6 per cent in the UK.

LUCKNOW: Amid the panic on shortage of Remdesivir injections, experts have pointed out that certain steroids and blood thinners were saving life of moderate to severely affected covid-19 stricken people.
Steroids and blood thinners are groups of medicines used for a spectrum of diseases. The two drugs operate differently in the body to counter medical problems. While steroids have anti-inflammatory properties, blood thinners are needed to prevent formation of blood clots in the body.
“Various strains of the Covid-19 virus attack the body and tend to give rise to conditions which cause inflammation and clot formation. If not managed well and on time, the patient’s life goes into serious danger which appears in the form of multi-organ dysfunction. We have noted that while Remdesivir helps in shortening the duration of illness, steroids and thinners are crucial to saving life of patients,” said Prof RK Dhiman, Director, Sanjay Gandhi Post Graduate Institute of Medical Sciences which is attending complicated patients.
He also called the panic behind Remdesivir an unwanted crisis and urged people not to hoard the medicine indicated for use in a small group of patients. “At our center, we are giving steroids and blood thinners to majority of the patients depending on their need. But largely. the success mantra to combat Covid-19 rests on steroids, blood thinners, supportive care and oxygen therapy along with the antibiotic cover,” he said.

Steroids like Dexamethasone (yes, its a steroid, most people are not aware of this!) were always more beneficial than Remdisivir but these steroids cant be given to every other person. Can not be given at all to diabetics which limits their use to much less people than Remdisivir.

India’s crosses a major landmark with more than 15 Crore Cumulative Vaccination Coverage


India has crossed a significant landmark in its fight against COVID19 pandemic today. The cumulative number of COVID19 vaccine doses administered in the country has crossed 15 Cr.

Cumulatively, 15,00,20,648 vaccine doses have been administered through 22,07,065 sessions, as per the provisional report till 7 am today. These include 93,67,520 HCWs who have taken the 1st dose and 61,47,918 HCWs who have taken the 2nd dose, 1,23,19,903 FLWs (1stdose), 66,12,789 FLWs (2nddose), 5,14,99,834 1st dose beneficiaries and 98,92,380 2nd dose beneficiaries more than 60 years old and 5,10,24,886 (1st dose) and 31,55,418 (2nd dose) beneficiaries aged 45 to 60 years.

Ten states account for 67.18% of the cumulative doses given so far in the country.



More than 21 lakh vaccination doses were administered in the last 24 hours.

As on Day-103 of the vaccination drive (28th April, 2021), 21,93,281 vaccine doses were given. 12,82,135 beneficiaries were vaccinated across 20,944 sessions for 1st dose and 9,11,146 beneficiaries received 2nd dose of vaccine.

Ten States including Maharashtra, Uttar Pradesh, Delhi, Karnataka, Kerala, Chhattisgarh, West Bengal, Tamil Nadu, Andhra Pradesh and Rajasthan report 72.20% of the new cases.

Maharashtra has reported the highest daily new cases at 63,309. It is followed by Karnataka with 39,047 while Kerala reported 35,013 new cases.



India’s total Active Caseload has reached 30,84,814. It now comprises 16.79% of the country's total Positive Cases. A net incline of 1,06,105 cases recorded from the total active caseload in the last 24 hours.

Eleven statescumulatively account for 78.26% of India’s total Active Cases.



The National Mortality Rate has been falling and currently stands at 1.11%.

deaths were reported in the last 24 hours.

Ten States account for 78.71% of the new deaths. Maharashtra saw the maximum casualties (1,035). Delhi follows with 368 daily deaths.





Six States/UTs have not reported any COVID19 deaths in the last 24 hours. These are D&D & D&N, Ladakh (UT), Lakshadweep, Mizoram, Tripura and Arunachal Pradesh.

I don't want to flood with Twitter links but the so called Indian Variant of the Chinese Virus is confirmed today in Germany, Finland, Uganda and now China.

If China is confirming that it is detected in several cities means that is it already circulating throughout China? Did it enter in China from multiple sources? Or is it spreading locally in China? Lots of answers needed.

Remember that flights are banned between India and China since last year.

What is the intermediate source to reach inside the heart of China?

Or is the Indian Variant so prevalent that it really went unnoticed in short period of time?

Media is doing a very fucktard job of creating panic that every patient is feeling they are gonna die without oxygen support.

Good job media..the merchants of death... creating panic and killing people.
Due to Panic created by Media only, people with mild symptoms are running to Hospitals and when told to go back are crying all over social media. I have got phone calls from so many of my friend asking to help arrange beds for their loved ones (few of my cousins are doctors) when I check their symptoms details and tell them better keep at home and follow the exercise to keep O2 level in body they cut the call.
I can confidently say that over 60-70% of beds would be occupied by mild to medium symptoms people and which is creating issue for moderate to high symptoms patients.

Why does Kerala regularly becomes the first place to find new mutants or new virus in India. People are moving to world from other Indian cities also but somehow always Kerala is where all these bad things starts!

Even a lot of my doctor friends are blaming the media for the rush in the hospital and the long line of queues.
The reason why many bystanders and people who rush covid patients with mild symptoms also get covid from the crowd is cuz of the panic created by the media rectums.
hospitals have become a hotspot cuz of the panic

Steroid use has very small window during acute phase with respiratory symptoms. Anti viral drugs to reduce the viral load again requires monitoring and viral load checks from Edta plasma extracted at 1500 rpms for 15 minutes on a standard centrifuge. The side effects can be over whelming with indiscriminate use.

Same is the case with anti coagulants.

IMO media should refrain from publishing such catchy articles. They can be misleading because the treatment of the patient is an evolving process where many supportive drugs have to be used to keep Patient Haemodynamically stable.

China is opened to both Bollywood and Hollywood despite no reciprocality. I think Chinese are simply less prejudiced and more open to culture from other places despite being commie. Not only movies but travel and overseas education too.
My cousin in America has a Chinese neighbour. That's it. He knows nothing else abt him except that he is Chinese. Dude, most of your nibas never even come outta your ghettos, let alone open up to foreigners and socialize in the states you live in. Learn a thing or two from Pakis. They atleast socialize, despite having the reputation of getting extremely fanatical at times. Expats tend to support their homeland, it is a natural thing. Still, I wouldn't accept your stance that Chinese are open to alien cultures.

India could soon have another locally developed vaccine

Drugmaker Cadila Healthcare, also known as Zydus Cadila, is conducting phase three clinical trials on 28,000 people, including those above 75 and children between ages 12 and 18, for its DNA-based vaccine candidate.




“We have completed the major recruitment for our phase three (trial),” managing director Sharvil Patel told CNBC’s “Street Signs Asia” on Thursday
