Regarding Nuclear threshold read what I posted earlier, how it starts is irrelevant(and yes a terror attack in main Indian cities is a good enough reason for crossing IB), India fought Kargil under the nuclear overhang and could have well crossed the IB without any consequences.

Sam are not 100 percent , still they can negate damage to great extent.
Plus , indian military should have a better SAM doctrine .
Especially in cashmere , so as to create a overlapping grid of such missiles , if it were there on 27 feb , 2019.
