New studies have come up explaining why some people end up with TTS (sometimes fatal) following ChAdOx-1 covid-19 vaccine (AZ vaccine/covishield). Instead of adenovirus vector being deposited in the muscle and than carried away via lymphatic system, some times inadvertent IV injection leads to vector adenovirus getting transported to spleen, followed by macrophage mediated Bcell activation and production of antibodies which activates platelets resulting in aggregation of platelet to form thrombus.
If you haven't taken the jab yet, make sure the nurse aspirate the plunger before injecting the vaccine to confirm IM deposition of vaccine instead of inadvertent IV delivery.

Aspiration of plunger is the most basic things a health worker learns while observing the injection procedures the first time.

Hard to believe people ignore basics like these.

I think it is mostly due to the belief that there is are no major vessels in the deltoid region, making it safe and default place for IM injections. There do exists some small vessels under the muscle and people with small muscle mass may get the inadvertent IV injection
