in China we have a compulsory medical insurance policy which both employees and employers must contribute to. now the govnmt's aim is to extend to backward or remote rural areas. but its coverage is only for mainly public hospitals

or one may choose to take out a commercial insurance policy

rather than giving 'free' the gvnmt may lower or 'waive' insurance policy for the 'poor'. certified hospitals anyway get remunerated from gvnmt sponsored insurance organization. when going to hospital patients settle payment with a 'social security card' while paying cash (or by credit card) in part on their own.

my point is to make it feasible and easy to manage.

Things will change for better once actual needy people say BPL will have their UID. Government has resources but Distribution & implementation system needs to be efficient. Once the needy mass is identified, Healthcare schemes will reach them with deeper penetration.

i am not sure about this thing being done on pan-india basis but the government of himachal has done precisely the same. they have insured the entire poor population of the state and has now handed out smart cards to the poor of the state, so each time they have a health problem all these people need to do is carry that card which amongst other information carries a thumb impression and make a thumb impression in a machine which has been installed in all the government hospitals in himachal. right from the cost of hospitalization to the cost of treatment all gets borne by the state government with not a single penny spent by the poor and all this money or part of it later on gets reimbursed to the state government.
