Even a fire on board a boat is reported. This was a massive accident which cost a lot to repair and also degraded operational preparedness. It has to be reported and not hushed away.

Worse is that the court of inquiry found both the captains guilty bu let off and one of them is right now on a trainin assignment.
INS Ranvir subsequently received a unit citation.

INS Kuthar...huh? what does Kuthar means in other Indian language? In Assamese it mean an Axe.

Training of the sailors no upto the mark..? Or the sailors not doing their job good enough ?

How freaking can you not see or detect two huge vessels on the path to collision..

One of the reasons for not too good on the job training and experience is the restriction on fuel and so the 'forays' into the sea is much less that desirable!

Could it be that the IN ships are now equipped with advanced sensors and electronics, and human presence on the deck is minimal? A lack of human watchers (or whatever the naval term for it is), and total reliance on electronic sensors/ radar can be a possible cause. Equipment failure, data misinterpretation or simple lack of training can cause such mishap - problem is how did this happen in two naval ships at the same time is still difficult to guess!

Orders are given to the helmsman by the Officer of the Deck (OOD). Ultimately the responsibility is the Captain's.

The officer of the deck under way has been designated by the commanding officer to be in charge of the ship, including its safe and proper operation. The officer of the deck reports directly to the commanding officer for the safe navigation and general operation of the ship, to the executive officer (and command duty officer if appointed) for carrying out the ship's routine, and to the navigator on sighting navigational landmarks and making course and speed changes. Also called OOD.
